We provide therapy as part of CCS care teams across Vernon, Crawford, La Crosse, and Monroe counties. CCS brings together a team of providers around your goals, not just a diagnosis, so you’re supported by more than one person on your care journey.
CCS is designed for people of all ages navigating mental health or substance use concerns that affect daily life – work, school, relationships, or independent living. It’s available to those enrolled in Medicaid within Vernon, Crawford, La Crosse, or Monroe County who are determined eligible through a county-administered screening.
As part of your CCS care team, we’ll work with you to match you with a provider best suited to your needs. Sessions are held at your home or a private confidential community setting.
